St. Charles County Youth Soccer Association (SCCYSA) wishes to provide an opportunity for SCCYSA Members to report to the Board of Directors any injuries, poor or unsafe field conditions, unsportsmanlike conduct, and rule or code of conduct violations. Incidents should be reported within 24 hours whether originating from physical actions, electronic communications, or verbal communications inside or outside of league play during the seasonal year. 

The League Incident Report (LIR) submission form is located on our website and must be completed through a Google Form submission after which the incident will be reviewed for resolution.  After review, the incident reporter will be contacted by the assigned SCCYSA Board member and the investigation process will commence.
